You can own an individual policy between the ages of 6 months and 69 years old. When getting a family plan, the children need to be between 15 days old and less than 18 years old. Children born into a family plan are “grandfathered” in.
You must be a Canadian citizen or landed immigrant who maintains a permanent residence in Canada. You must read and speak English or French, and be between 18 years and 69 years. you must also answer NO to the following questions:
Have you or the person’s applying for a policy received any medical advice, been treated for or diagnosed with any of the following in the last 5 years?
1. . Heart attack, angina, atrial fibrillation, congestive heart failure,
coronary artery disease that required bypass surgery, a stent or
angioplasty, or a heart valve replacement?
2. Stroke, transient ischemic attack (TIA)?
3. Cancer, melanoma, Hodgkin’s disease or leukemia?
4. Multiple Sclerosis, Cystic Fibrosis, Muscular Dystrophy or
5. Alcoholism or drug addiction?
6. Diabetes which requires insulin, or Diabetes which was
diagnosed prior to age 40?
Has anyone applying for coverage ever had any positive test, treatment for or exposure to Human Immunodeficiency Virus (HIV) or Acquired Immune Deficiency Syndrome (AIDS) or AIDS Related Complex (ARC)?

At the age of 25 years a child in the family plan will be able to convert their plan to an individual plan. Even if they develop a health condition that would not otherwise qualify them for this plan, they are pre qualified to own it individually and be covered until age 85.
Yes! Children who are born into a family plan, are grandfathered in at birth. For example, a premature baby who ends up in NICU for 20 days and in a hospital for 60 days – the benefit for the family would be up to $96,000. (Base plan would be up to $24,000)
No. The company cannot increase your premium due to the number of claims you file.
The policy terminates at age 85.

Daily Hospital Benefit for up to 365 days per sickness. Benefit between $75 and $300 per day can be chosen. (In $25 increments) Premium is adjusted depending on the benefit chosen, and the age of the applicant as well as whether is it is an individual or family plan.
Daily Intensive Care Benefit for up to 365 days per sickness, paid IN ADDITION to the daily hospital benefit. Benefit between $300 and $1200 depending on plan chosen.
Daily Recovery Benefit following Hospital confinement of a minimum of 10 days – where you can recover at home or at work. When hospitalized for more than 3 days, recovery is up to 3X the number of days hospitalized.
Outpatient Surgery Recovery benefit of up to 14 days. Top plan pays up to $4200
Ambulance Reimbursement between $150 and $600 depending on plan you choose.
Premiums are based on the benefit selected at time of application, and are adjusted according to age. The older you are the more at risk and therefore the higher the premium. It is best to “lock in” the premium as early as possible when you are healthy! The premium will also depends as well as to whether it is an Individual or a Family plan.
It depends on the condition. If it would cause you to say YES to our 7 qualifying questions then you would not be eligible for coverage. However, if you do qualify, then any condition you are being treated for in the last 12 months, will be covered after the 2 year period. All other sicknesses outside of pre existing would be covered from the day the plan is issued.
Pre authorized withdrawal from a checking account is set up during the application process for the initial premium which comes out within 3 – 5 days of application submission and the recurring premium which comes out on the selected preferred billing date.
